Missing from the letter

Holes: Named the bank you are writing to, Legal entity, DBA, and principals, Asked for the listing ICA, Asked for the reason code, Asked who added you, and on what date, Invoked the 7-day question window, Invoked the 30-day removal clock, Attached the termination or decline.
